1. Fees in this Practice are according to the guidelines set out by the South African Medical Association (SAMA).
2. The Patient/Medical Aid Member is personally responsible for paying Dr Fraser’s fees.
3. Terms of payment: All consultations are due for payment at the time of consultation. Accounts involving surgery must be settled in full at the first post-operative visit i.e two weeks after the date of service.
4. The Patient/Member is responsible for submitting the account to their Medical Aid.
5. The benefits due to you are based on your Medical Aid Policy and the specific Plan to which you belong.
6. Dr Fraser’s fee will be more than the NHRPL (National Health Reference Price List) which was unilaterally determined by the Dept of Health.
7. The Practice reserves the right to charge interest on accounts that are more than 30 days outstanding.
8. The Patient/Medical Aid Member will be liable for any legal costs incurred in recovering outstanding monies.
